Required Forms for All Employees

Form I-9 Employment Eligibility Verification

Form I-9 verifies the identity and employment authorization of individuals hired for employment in the United States. An employee must attest by completing section 1 of the Form I-9 as well as by presenting their employer with acceptable documents as evidence of identity and employment authorization. 

Completing Form I-9 Online
  • The I-9 form must be completed no later than the first day of employment.   
    • Newly hired employees will receive email notification to complete the Section 1 of the form electronically.
Acceptable Documents

Employees are required to report to the Human Resources office to submit original documents that verify their work eligibility, prior to or on their first date of employment.  

One or two forms of identification that establishes identity and employment authorization are required.

Employees whose U.S. Citizenship status is classified as a noncitizen authorized to work must also present additional documents to prove their work authorization in the United States.
