The following health requirements must be submitted prior to beginning upper division nursing classes. You are encouraged to begin collecting the following data, certifications, immunizations and/or titers (Childhood immunization records are accepted). If admitted to an apprenticeship program, these requirements may be handled by the practice partner.
On-campus health services are provided by the Student Health Center.
NOTE: All required health documents will be submitted to PreCheck/SentryMD.
MMR - Measles, Mumps, Rubeola Measles
- Immunization with two (2) doses of vaccine required or Titer results demonstrating immunity is required
Tetanus, Diphtheria, Pertussis (Tdap)
- Tdap booster required every 10 years after initial Tdap on file. May never be more than 10 years old during enrollment.
Hepatitis B
- Immunization with three (3) doses of vaccine required or Titer results demonstrating immunity is required
- Verification of the first two Hepatitis B immunizations must be submitted by the deadline
Varicella (Chickenpox / VZV)
- Immunization with two (2) doses of vaccine required or Titer with results demonstrating immunity is required.
Flu Shot
- Fall requirement of each year in upper division nursing courses
Tuberculosis (TB / PPD/ Mantoux tuberculin (M) Screening Test
- (2-step) 2 separate TB skin tests or (1-step) TB blood test is required
Physical / health examination / Essential Functions (form to be provided)
CPR Certification by the American Heart Association
- CPR must be "BLS Healthcare Provider" from the American Heart Association
Health Insurance
- Copy of insurance card front and back is required and name of Policy Holder / Subscriber.
- If you obtain UAH / United Health Care insurance the card or confirmation must show the active semesters, for example: 2018-2019
- Student insurance may be obtained from United Health Care Insurance through the Student Health Center Health Insurance page.
