It’s not too late to earn your Charger Fit Wellness Incentive! The deadline to complete the biometric screening and health assessment for the 2023 Wellness Program is quickly approaching. October 31st is the last day to complete your mandatory components and earn the 400 points required to receive the end of year incentive. Biometric Screening
To complete your biometric screening, you may schedule an appointment with the Faculty/Staff Clinic or you can have the screening completed with your own physician. If you decide to use your personal physician, to receive credit you must have your physician complete the biometric screening form and provide it to the Faculty/Staff Clinic. Email chargerfit@uah.edu or visit the Charger Fit website to obtain the form. BONUS: A $50 gift card will be emailed to all eligible employees who complete the screening!
Health Assessment
To complete the health assessment, login into your Wellness Center at hchealthbenefits.com or via the HealthComplete app: click on the health assessment tile to complete and submit your assessment.
In order to earn the $150 wellness incentive at the end of the year, you are required to complete the biometric screening and health assessment . Don't delay, get the biometric screening and health assessment completed and earn your activity points before the deadline runs out.
